Many young people don't know:

• how Medicare or the health system works
• what information they need to bring to appointments

• how to find appropriate adult specialists.

Without support, the transition process can result in disengaged patients who fall through the cracks of our healthcare system. Our research and experience has shown that having a structured transition process is beneficial, and importantly, non-clinical practice managers and administrators are perfectly positioned to lead this process.

Jacqui Ward
Manager - Community Engagement @ Eltham Paediatrics

Jacqui is the co-founder of Eltham Paediatrics and, in 2020, developed an admin-led transition process in response to questions from older patients and in recognition of the need for support and preparation for young people as they transition into adult healthcare.

The process was developed through research, interviews and, most importantly, input from young people themselves about what they actually wanted and needed.

Jacqui's approach combines practical systems thinking with genuine care for patient outcomes. This workshop reflects the same philosophy that guides Eltham Paediatrics: expert care combined with practical plans.

Jacqui holds a Master Degree in Social Administration (Community Development) and has more than 25 years’ experience in community engagement.
