Heart failure (HF) is a serious chronic condition that affects many Australians and requires lifelong management . HeartPlan is an education and support program to assist in the identification and management of patients with HF. It provides primary care nurses skills, knowledge and tools on the evidence-based management of Heart Failure.
This series of online workshops / webinars aims to:
1. Provide practice nurses across the Hunter New England and Central Coast regions with five (5) hours of education on HF
2. Improve practice nurse skills and knowledge regarding HF guideline-based therapy
3. Promote professional relationships between acute and primary care to support patient management
