Health Literacy for Team Leaders and Case/Care Managers

About this event

Why is health literacy important?

Embedding health literacy into home care is one way of minimising the risk of potential harm. Our experience is that where Team Leaders and Case/Care Managers have sufficient health literacy skills, consumers experience higher satisfaction with their care and services.

This is supported by the research findings of Palsey and Jakimowicz (2019) who found clear links have been established between low health literacy (HL) levels and poor health outcomes.  Home Care Team Leaders and Case/Care Managers are a key source of information and capacity building in regards to health literacy. They have a key role in supporting vulnerable consumers and their families with healthcare information.

This webinar will provide knowledge, understanding, skills and confidence to improve the health literacy of care managers, consumers and frontline workers. At the end of the session, you will know why health literacy is important and how to use some simple techniques to improve how you communicate health literacy. You will also learn practical strategies for integrating health literacy into systems, and processes and enabling the workforce to contribute.

Learning Outcomes

By the end of this webinar course, you will be able to:

✔ Define health literacy and the benefits to consumers and their families

✔ Describe and use health literacy tools, e-Health and techniques

✔ Identify factors that influence health literacy and the impact of health literacy on consumer's health choices

✔ Create a personal action plan to be health literacy friendly

✔ Outline strategies to integrate health literacy in systems and processes

✔ Identify strategies to equip frontline workers to contribute to health literacy

All attendees will receive a Statement of Attendance upon completion of an assessment as evidence of Continuing Professional Development hours.

Who will benefit most from this webinar?

Care/Case Managers, Team Leaders, clinicians who are supervising care staff and non-clinicians who want to learn more about the importance of health literacy.

Benefits to providers and consumers

Improving the health literacy of consumers is likely to improve health and care outcomes and reduce the risk of deteriorating conditions and unplanned hospitalisations. By equipping the workforce to support health literacy, consumers will be empowered and equipped to maximise their wellness and health outcomes.
