Overview
This Education is Intended for:

Presented by:
Dr. Alison White
Overview
This education utilises an evidence-based framework to equip health professionals with the communication skills necessary for effectively engaging with patients facing life-limiting illnesses and their families. Goals of care conversations and planning evolve throughout the patient's journey, from admission to discharge or end-of-life, and should never occur in isolation. Effective communication of patient values is essential among all healthcare professionals, both within individual teams and across multidisciplinary settings. Participants will gain an understanding of how to incorporate patients' values and preferences into care plans, ensuring that treatment is both compassionate and person-centred.
The in-person education features simulation-based learning, where participants engage in role-play scenarios with professional actors as the simulated patient. These realistic scenarios provide opportunities to practice and refine communication techniques. A Communication Facilitator provides real-time feedback, guiding participants in navigating patient values and collaboratively developing tailored treatment plans through shared decision-making.
Learning areas
1.Participants will learn to apply the evidence-based framework, ICE-REMAP to support them in their goals of care conversations with patients.
2.Participants will learn the importance of identifying and incorporating a patient's values and preferences into the development of an effective treatment plan.
3.Participants will engage in a role-play with a simulated patient, practicing the ICE-REMAP framework. They will receive feedback from both the simulated patient and the Communication Facilitator to build confidence in using the framework.
4. Participants will develop a personal script to incorporate into their Goals of Patient Care discussions from insights they gained from the role-play and self-reflection.
