Emergency Care Planning (Cairns)
Are you the primary caregiver for another person? Do you have an emergency care plan?
An emergency care plan helps ensure there is continuity of care. This is a crucial document that gathers all essential information about a person's care needs in one place, enabling another person to step in quickly if you are unavailable to fulfill your caring responsibilities due to illness, injury, or another emergency. It contains details such as medical contacts, medications, specific daily routines, and emergency contacts. This plan acts as a blueprint if an unexpected situation arises, preventing uninformed decisions and ensuring your person’s wellbeing is being fully considered.
Join the Carer Gateway team for a 2-week coaching group where you can complete an emergency care plan for those you care for and discuss how you could overcome any obstacles for continuity of care should an emergency occur. To optimise the information in your plan, bring details of any medications the person you care for takes, as well as the details of all their health providers, medical teams, and any other support services you use. We will also explore who you could give a copy of your plan to, as well as the importance of completing a carer emergency card to keep with you.
Sessions will be held at 1pm-3pm on Tuesdays 17th and 24th March, 2026. Snacks and light refreshments will be provided. Please include details of any food allergies or other special dietary requirements with your RSVP.
