AIHI Seminar: How can we learn from work that goes well? Or what happens, when "nothing" happens?

TheAustralian Institute of Health Innovation(AIHI) is a leading research institute investigating many aspects of healthcare. AIHI is comprised of four Centres: the Centre for Healthcare Resilience and Implementation Science, the Centre for Health Informatics, the Centre for Health Systems and Safety Research and the Centre for the Health Economy. We’re a major national resource for many people whose work is intrinsic to strengthening health systems, organisations and services.

<H3>Abstract:</H3>
<P CLASS="MsoNormal"><SPAN>The current approach to patient safety (Safety-I) relies on a traditional ‘find and fix’ approach. The purpose of this approach is to find the causes of what has gone wrong, after the event, and to try to eliminate or contain them in order to ensure that the number of failures is as low as possible.</SPAN></P>
<P CLASS="MsoNormal"><SPAN>Healthcare is, however, far too complex for such a linear model approach to be sufficient. This realisation has led to a change in focus to Safety-II: a concerted effort to enable everyday clinical work to go well rather than to prevent it from failing. </SPAN>The key is to appreciate that everyday performance succeeds far more often than it fails because people (nurses, clinicians, and managers alike) always adjust what they do to match the conditions.</P>
<P CLASS="MsoNormal">The emphasis of Safety-II is to facilitate work flexibility and actively try to increase the capacity to deliver more care more effectively. Most importantly, resilient health care and Safety-II focus on how everyday performance usually succeeds rather than on why it occasionally fails, and actively strives to improve the former rather than simply preventing the latter.</P>

<P CLASS="MsoNormal"><SPAN>Professor Erik Hollnagel, PhD, </SPAN><SPAN>is Senior Professor of Patient Safety at Jönköping University, Sweden. He is also Visiting Professor at the Centre for Healthcare Resilience and Implementation Science, Australian Institute of Health Innovation, Macquarie University, Australia, and Professor Emeritus at the Department of Computer Science, University of Linköping, Sweden.</SPAN></P>
<P CLASS="MsoNormal"><SPAN> Through his career, he has worked at universities, research centres, and industries in several countries and with problems from many domains including nuclear power generation, aerospace and aviation, software engineering, land-based traffic, and healthcare. His professional interests include industrial safety, resilience engineering, patient safety, accident investigation, and modelling large-scale socio-technical systems.</SPAN></P>
<P CLASS="MsoNormal"><SPAN>He has published widely and is the author or editor of 24 books, including four books on resilient health care, as well as a large number of papers and book chapters. The latest titles are “Safety-I and Safety-II: Developing the resilience potentials”, and “Delivering Resilient Health Care” co-edited with Professor Jeffrey Braithwaite and Professor Bob Wears. </SPAN></P>
