Join us for a FREE informative workshop hosted by Palliative Care Western Australia working on the Advance Health Directive doucment.
If you have completed the My Future; Care Advance Care Planning workshop, you can attend the Advance Health Directive (AHD) document support workshop that will assist you to complete your AHD.
During this workshop we will work through each page of the AHD allowing you to work towards the completion of the document to a stage where it is ready for signing , witnessing and presenting to your health professional.
Participants should have considered their end-of-life wishes, values and preferences and should be ready to complete their document in preparartion for attending this workshop.
