If the time comes that you are unable to communicate your wishes, how would you let those who care for you know exactly what you want? Do you have an Advance Care Directive?

An Advance Care Directive (ACD) empowers you to:

  • make clear legal arrangements for your future health care, end of life, preferred living plans and other personal matters
  • appoint Substitute Decision-Makers to make informed decisions on your behalf if you are unable to make them yourself

We are providing resources and supporting residents understand how to complete an Advance Care Directive through a free information session. Book your spot now as numbers are limited.

Please note that this is NOT an event where already completed ACDs can be witnessed or certified. This session is based around sharing awareness and information in a welcoming environment with interactive resources with facilitators.

Frequently asked questions

Do I need to bring an ACD form with me?

No, we will provide all the resources for FREE on the day.

Will I complete the ACD form during the workshop?

The workshop helps you understand the form, start to draft your answers and have your queries answered. We do not recommend completing it within the workshop but provide everything you need to take away and continue the process.

Can I bring a friend or partner?

You are welcome to bring friends and partners to this event, but make sure you reserve a ticket for everyone who plans to come as numbers are limited.
