An Advance Care Directive (ACD) empowers you to:
- make clear legal arrangements for your future health care, end of life, preferred living plans and other personal matters
- appoint Substitute Decision-Makers to make informed decisions on your behalf if you are unable to make them yourself
We are providing resources and supporting residents understand how to complete an Advance Care Directive through a free information session. Book your spot now as numbers are limited.
Please note that this is NOT an event where already completed ACDs can be witnessed or certified. This session is based around sharing awareness and information in a welcoming environment with interactive resources with facilitators.
