A Personal, Portable Summary: For Us and the Person We Support (Online)

1-hour, free, online information session for mental health carers.

Do you have to repeat the same details about the person you support every time you meet a new clinician or service provider? A personal and portable summary can make your life easier.

In this free session, you’ll learn how to create a simple, adaptable document to keep track of important information—like medical history, medications, key contacts and support needs—so you don’t have to start from scratch every time. Consent is a critical part of this prior to sharing.

You’ll walk away with:

• a ready-to-use portable summary template
• tips on what to include and how to keep it updated
• a practical tool to reduce stress and ensure accurate information is shared.

This information session is designed specifically for family members, kin, young carers, friends and other individuals who support someone experiencing mental ill-health. We are a Queensland-based service.
