• Clinicians who are from services newly registered with PCOC
• Clinicians implementing PCOC into routine clinical practice

Objectives

At the end of this workshop participants will have an enhanced understanding of:

• Assessing the palliative care phase
• Assessment tools for symptom and functional assessment
• Using assessment data to inform care and improve practice
• Identifying strategies for integrating PCOC assessments into practice

Prerequisite

Services that are registered with PCOC

Completion of the PCOC Essentials online module
