<p>Come along and learn how our new CARE Service is using a combination of healthcare, in-home assessment and a digital command centre to meet the needs of older people requiring non-emergency care. We are taking advantage of the latest digital enhancements in health care via a:</p><ul><li>Virtual Command Centre - a digitally enhanced command centre that allows collaborative triage and prioritisation between SAAS and SALHN to support patient care in the most appropriate and timely place. </li><li>Eyes-on-Scene Outreach Team - who deliver a combination of telehealth and in-home assessment and intervention to patients where appropriate, reducing SAAS transport requirements and demand on EDs.</li><li>CARE Centre- a six-treatment space facility at the Repat Health Precinct specifically designed to meet the needs of older people requiring non-emergency but comprehensive assessment and treatment in a more appropriate environment to EDs. </li></ul><p></p>
