Last Days is a program to equip you with practical knowledge and tools to support a person living with dementia as they approach the end of their life.

Our experts will help you understand death and dying and plan for what’s ahead, connecting you to support and others going through the same experience.

Planning for what’s ahead

Last Days helps you understand what’s ahead when someone in your care is at the end of their life, creating space for open conversations about their wishes and preferences and giving you practical knowledge to help you plan and prepare.

The workshop will cover:

  • What to expect when we die and the role of palliative care
  • What a “good death” is and the choices we have
  • How to have conversations about dying and plan for what’s ahead
  • Practical matters to get affairs in order
  • Care choices as we near death
  • Grief and loss
  • Rituals, rules and regulations around death

Drawing on decades of research and experience in complex dementia and palliative care, the program is delivered by an experienced palliative care nurse, in a small group face-to-face environment. Participants receive a comprehensive workbook including links to local services.
