This 45 minute introductory session is hosted by Palliative Care Tasmania.
We will talk about what Palliative Care Tasmania does and how this differs from (and how to access) the Specialist Palliative Care Service.
Palliative care focuses on providing the best quality of life for anyone of any age, living with a life-limiting condition. It identifies a person’s physical, emotional, spiritual and social needs.
Palliative care can be provided at home, hospital, aged care facility or anywhere you live. The team of carers may include your GP, aged care worker, community nurses, other medical professionals and family and friends.
We will also provide an introduction to the Tasmanian Advance Care Directive. Making an Advance Care Directive (ACD) allows you to decide now, or to guide, what health care and treatment you receive, in the future, if you lose the ability to make and communicate such decisions yourself.
Palliative Care Tasmania (PCT) is Tasmania’s peak body for palliative care. Our vision is that all Tasmanians with a life-limiting illness, together with those they value, are supported to live, die and grieve well.
