Target Audience: Participation is open to any clinician, team leader and manager representing services that are interested in progressing a consumer-directed care approach within their organisations across inner north western Melbourne.

The Western Melbourne Cross-Sector Community of Practice (the CoP) is a network of health, community and disability service providers and advocates, delivering supports to clients in the western Melbourne metropolitan areas.

The CoP is an initiative of the Ageing, Disability, Mental Health Collaborative Panel, and brings together different sectors to collaborate and exchange good practice to achieve better consumer-directed care outcomes.
