For GPs

In this education session, presenters Professor Visvanathan, Mr Thompson and Ms Dobie will inform attendees how to identify older patients within the general practice setting who are at risk of frailty. The presenters will also provide an update on the management options available for patients who are at risk and discuss appropriate referral pathways into community support services.

LEARNING OUTCOMES

  • Describe the concept of frailty and its impact on the wellbeing of older patients;

  • Perform screening and asessments for patients who are at risk of frailty;

  • Explain the benefits of exercising, nutrition and socialising in managing frailty;

  • Describe the support services available and appropriate referral pathways for various community aged care services.
