Why is clinical care in the home important?
The model of care in the home for aged persons has significantly changed in the last 5 years. Since the introduction of the Aged Care Quality Standards and a general increase in the complexity of care, clinical care is even more important to all home care providers.
Monitoring consumers' health conditions and responding in a timely manner is now a fundamental requirement for all service providers. It is therefore vital for the workforce to be trained in clinical care appropriate to the scope of their practice.
Overview
This six-module course has been designed to staff who are working in-home care to be better educated and informed about their responsibilities when providing and monitoring health care to consumers. Attendees will be given case scenarios to consider and extensive resources to use in the workplace.
The two-session course will cover the elements of clinical governance and how to implement; recognising deteriorating conditions; high prevalent risks; what is ‘scope of practice’; safe medication management; validated assessment tools; managing clinical referrals and the role of the direct care worker in the continuum of care. In addition, the importance of meeting the Aged Care Quality and Safety Standards and NDIS High-Intensity Supports will be highlighted.
This course will also equip attendees with the knowledge and skills to conduct assessments, as well as develop and implement care plans. The course will highlight risks of non-compliance and the development of a risk-based internal audit schedule. This course has been revised and updated for 2022 to reflect contemporary practice and information.
Learning Outcomes
By the end of this course, you will be able to:

✔ Explain scope of practice and the process of referrals
✔ Recognise and respond to deteriorating conditions and high prevalent risks
✔ Manage medication safely in a home care setting
✔ Conduct assessments using validated assessment tools and develop care plans including clinical care
✔ Identify and manage risks of non-compliance and determine a risk-based audit schedule
✔ Understand Duty of Care and Dignity of Risk
Recognition
All attendees will receive a Statement of Attendance upon completion of an assessment as evidence of Continuing Professional Development hours.

Clinicians and care managers who are supervising care staff and non clinicians who want to learn more about the importance of safe clinical care provision. For example coordinators, service managers, quality managers, educators, supervisors and team leaders.
