In conjunction with Glen Sorensen from Age Communications, we will be hosting a workshop to share learnings from the successful pilot project to support short-term reablement, The Illawarra Way.

This workshop is specifically for CHSP service providers, RAS staff and those working in health and allied health.

In the workshop, we will cover:

Background:

– Wellness and Reablement Approaches

– Types of reablement

– Lessons from the Illawarra Way Trial of a System for Short-term Reablement


The Illawarra Way System for Responding to Short-term Reablement Referrals from RAS:

– RAS roles, responsibilities – identifying reablement potential, gaining consent and making referrals

– CHSP roles and responsibilities – creating reablement capacity and accepting referrals

– Support worker roles and responsibilities


Dealing with Challenges:

– Negotiating coordination and case management responsibilities

– Extending reablement periods

– When ongoing support provision is sought


About Glen Sorensen

Glen is the Managing Director and Principal Consultant of Age Communications. He is an experienced and accomplished facilitator, trainer, writer and researcher who has worked extensively with the aged care sector. He is also the author of When I'm 64, Millennium Books and Options for the Ageing: the complete guide to support and care of older people, Simon and Schuster. He is the presenter of the MDS Training DVD produced by Ageing, Disability and Home Care, NSW Department of Family and Community Services. He is also the writer and presenter of the 2016 DVD Walk the Person Centred Talk, a practical resource on the person centred approach in work with consumers.
