The Grace Hornsby Open Day

This is a face to face event whereby you will have the opportunity to view The Grace Hornsby apartments. The apartments will be open on Wednesday 10 August 2022, please book a time by clicking 'Select a Time' on the right hand side of this page.

Nestled in the quiet suburban area of Sydney’s Upper North Shore, The Grace is perfectly positioned across the road from the Asquith Bowling Club. In close proximity to Asquith train station, local shops, Storey Park and Hornsby Westfield, this development includes all the modern-day conveniences.

The beautifully presented single-level apartments have been designed to achieve Platinum level certification under Liveable Housing Design Guidelines and meet High Physical Support design requirements under the NDIS.

You will learn about:

• SDA provider Summer Housing
• The design, features and model of support within the apartments
• Who is likely to be eligible for these apartments and how to apply
• The SDA pathway for NDIS participants

Who should attend?

• People with disability
• Support coordinators
• Case managers
• Social workers
• Discharge planners and allied health professionals
• Family members and supports of people with disability
• Anyone working with people who may be eligible for SDA

There will be an opportunity to ask questions on the day about SDA and current NDIS processes for seeking SDA funding.

We will email you to confirm details once your registration is complete.

All visitors to Summer Housing properties must be vaccinated or be able to present a medical exemption certificate. Our team will contact you prior to the day to discuss your status.
