  • The Connecting End of Life Care in North Queensland (CELC-NQ) project aims to improve the capacity of Residential Aged Care and General Practice service providers within the North Queensland region Hospital, to provide quality palliative care and end-of-life planning thereby assisting in the management of after-hours service need.
  • This is be achieved by building culture, systems and practice improvements which increase the quality of end-of-life care
  • This Practice Development Program has been designed in response to the results of a Residential Aged Care Training Needs Analysis and best practice palliative care – for more information - https://palliativecareqld.org.au/celc_nq/
  • CELC Northern Queensland Projects are administered by Palliative Care Queensland supported by funding from the Australian Government through the PHN Projects.
