Supporting consumers with complex care needs in the home
About this event
Why are complex needs important to manage?
Are you sitting on a 'risk' time bomb? What does it mean to have complex care needs?
What are the three common risks to client safety and wellbeing in a home care environment?
What are high impact, high prevalence risks in aged care?
Unless consumers with high-prevalent, high-impact risks and high-intensity supports are being managed appropriately, your service may be at risk! A common factor for many instances of non-compliance with the Standards is poor management of consumers/clients who are at a greater risk.
It is essential that providers have structured systems and processes in place to ensure risks are identified and appropriately managed to eliminate, minimise or manage the risks. This must start with an effective clinical governance framework including setting clinical indicators and regular monitoring.
It should include operational policies and procedures and documentation. It should also flow through to care support workers who have adequate training to observe and report key changes or risks.
At LPA our team of experienced clinicians can support you so that you have effective systems in place to manage high care consumers/clients.
Do these statements truly reflect your everyday practice?
- Do you need a specific approach for 'at-risk' consumers/clients?
- Do you want guidance on what your policies and procedures should include?
- Would your clinical and care management staff benefit from understanding care planning for those at-risk?
- Would you like guidance on clinical indicators and monitoring?
If you answered YES to these questions, we have solutions for you...
