Description

Social prescribing is a non medical referral made by a health professional (GP's, Allied Health workers) to a Link Worker who co-designs a health and well-being plan with the patient that utilises the community and voluntary sector in order to improve the patients health and wellbeing.

Hear from Professor Katherine Boydell (Black Dog Inst), David Menzies, Chronic Disease Program (SEM PHN) and Nicole Battle, CEO, Neighbourhood House Vic), as they discuss social prescribing and its importance in improving health and well-being outcomes. Facilitated by Maxine McKew.
