Turning Point has been contracted by the Department of Health and Human Services to provide state wide training in Service Integration to the alcohol and drug sector.
People experiencing alcohol and drug problems may also have a range of other needs. These can include physical and mental health needs, as well as housing, employment and other social care needs, including relationships and parenting. It is essential that AOD workers have the skills and knowledge to work with a range of agencies to support clients in their recovery.
This one day workshop will suit all workers in the AOD sector and aims to enhance good practice in relation to the development of joined-up service planning for vulnerable clients with multiple service needs. Strategies will focus on the delivery of whole person, integrated and person-centred care.
This workshop provides an opportunity for information sharing, skill development and reflective practice.
The workshops will be held in three (3) metropolitan and four (4) regional areas allowing participants to consider issues in a local context.
This interactive workshop is both practical and skills based and will cover the following areas:
Identifying and reflecting on Care Coordination skills
Reviewing Inter-agency relationships
Identifying strategies to strengthen relationships with both AOD and Non AOD services
Joint care planning
Client centred approach
Implementing effective referrals
Sharing of client information
Community linkage and long term recovery planning
Use of Optional Modules to support integrated care planning
Review and monitoring client progress
This workshop will be facilitated by Sharon Patterson who is a Senior Education and Training Officer from Turning Point. Sharon has had extensive involvement in Care and Recovery Coordination through managing the Care and Recovery Coordination Community of Practice.
Co facilitator Jacqueline Langrick is an Education and Training Officer who has a Social Work background. Her work has often required working in conjunction with other services around clients with complex needs.
