Central Coast Local Health District (CCLHD) is providing an information morning for Service Providers and Community Members in the Better Health - Chronic Disease Self-Management Program (CDSMP). The CDSMP was developed by Stanford University and has an established record in strengthening self-efficacy and self-management across a range of conditions.
The infomation morning is designed to give you an understanding of the effectiveness of the "Better Health Self-Management" (BHSM) workshop. This workshop series has proven to improve the motivation and health of people living with ongoing health conditions such as Arthritis, Diabetes, Lung conditions, Heart conditions or Multiple sclerosis just to name a few.
We're hoping you'll like what you see and hear and register to become workshop leaders within your own organisation under our CCLHD Licence with Stanford University. The training will accredit participants as "Better Health Leaders" for the internationally recognised chronic disease self-management workshop series. As an accredited self-management program leader you will be able to:
- Improve client interactions about their personal motivation and health issues
- Develop client centred goals and plans
- Deliver the Better Health Self-Management workshops for organisations with a CDSMP Licence
The training will enable you to help people develop skills in self-management including:
- how to manage chronic disease symptoms
- the benefits of healthy eating and physical activity
- how to communicate effectively with your doctor and healthcare team
- how to manage difficult emotions
- how to make daily tasks easier
- how to set reachable goals
- how to get more out of life
