The topic
A Goals of Patient Care form has been developed to provide a clear, consistent and effective approach to establishing the goals of medical care for patients admitted into WA hospitals who are also experiencing clinical deterioration. Introduced last year, this document aimed to prompt and facilitate proactive shared decision making between treating clinicians, patients and their families.
Presented by Dr Simon Towler, this sector forum will provide an update on the Goals of Patient Care project, looking at how it is working in our hospitals, what the interaction with patients and their families has been like to date, and how the National Goals of Care Collaborative is progressing.
The speaker
Dr Simon Towler has expertise in all aspects of intensive care and is currently a specialist in intensive care at Fiona Stanley Hospital. As the Clinical Lead for Goals of Patient Care, Simon also works with WA Health and the Australian Digital Health Agency on the National Goals of Care Collaborative. This group aims to increase the use of My Health Record for storing and facilitating ease of access to a range of end-of-life planning documents.
The location
There are two attendance options available:
- In-person at the Seminar Room, Milroy Lodge, 15 Bedbook Place, Shenton Park, where light refreshments will be provided. (limit of 30)
- Online via Zoom .
