Free Advance Care Planning and Palliative Care workshop for GPs, Practice Nurses and Practice Managers to create better primary health care

About this Event

Central Queensland, Wide Bay, Sunshine Coast PHN invites GPs, Practice Nurses and Practice Managers to attend the Advance Care Planning and Palliative Care education evening. This event will focus on better primary health care through team -based initiation of advance care planning and palliative care.

What is the Advance Project?

It’s a free, evidence-based toolkit and training package specifically designed to support GPs, nurses and practice managers to initiate advance care planning conversations and assess patients’ and carers’ palliative and supportive care needs.

What is the purpose of the project?

The purpose of the project is to build capacity of general practices and primary care clinicians to provide better care through team-based initiation of advance care planning and palliative care.

Who is behind the project?

The Advance Project is funded by the Australian Government Department of Health with training endorsed by the Australian Primary Health Care Nurses Association (APNA) and accredited by the Royal Australian College of General Practitioners (RACGP) and the Australian College of Rural and Remote Medicine (ACRRM)

What are the Learning Outcomes:

  • Utilise the Advance ProjectTM Guide and resources to identify patients suitable for an:
  • Introduction to advance care planning (ACP)
  • Assessment of palliative/supportive care needs + introduction to ACP
  • Utilise the Advance ProjectTM evidence-based resources to introduce ACP and assess patients’ and their carers’ palliative and supportive care needs
  • Evaluate patient’s needs for follow up regarding ACP and/or any additional support required to ensure their palliative/supportive care needs are met
  • Work as a team to implement the Advance ProjectTM resources in your practice

Who is the Key Presenter:

Kelly Arthurs, Registered Nurse

Kelly Arthurs is the Palliative Care Clinical Nurse Consultant, HammondCare Centre for Learning & Research in Palliative Care. Kelly is also the Project Lead for Northern Sydney Residential Aged Care Home End of Life Project.
