This workshop will focus on strategies and tools to support staff to monitor and review Goal Directed Care Plans (GDCP). This will include discussion about:
how GDCP reviews can be completely efficiently and effectively within different settings
conversation skills and strategies to support effective GDCP review
documentation requirements for GDCP review
indicators for reassessment.

Pre-requisite training: This workshop is designed for staff who have attended introductory GDCP training (such as Developing effective GDCP or GDCP: Why, When and How!) and are actively involved in completing and reviewing GDCP with their clients. If you are unsure if you have completed the relevant pre-requisite training, please contact us at firstname.lastname@example.org
What to bring: Please bring at least one completed care plan that you have created with a client. This can be documented on any form / template. Participants who aren’t responsible for creating care plans (e.g. program managers) should please bring a care plan that has been created by another member of your team.
Catering: Morning tea OR afternoon tea will be provided (please enter dietary requirements when booking your ticket)

Statement of Attendance: Participants will receive a statement of attendance at the end of the session.
Fees and invoicing: Registration fees include provision of all training materials and catering. 10% GST and the Eventbrite booking fees are included within the ticket price. A tax invoice will be emailed to you following registration.
Cancellation fees: There is a $50 cancellation fee up to 28 days before the workshop, $100 fee for cancellations made 7-27 days before the workshop. No refunds will be provided for cancellations made less than 7 days before the workshop commencement.
