This is part of a series focused on providing palliative care education for staff working in residential aged care.

Target Audience: All clinical and management staff in residential aged care homes.

Education Overview:
Aged care staff play a vital role in supporting residents in their later years. Recognising and responding to deterioration in a resident’s condition is an essential clinical skill that enables clear communication with families, carers, and staff.

Accurately identifying deterioration ensures that residents receive high-quality, person-centred care while enabling staff to provide support aligned with a resident’s values, preferences, and goals.

Timely recognition and response minimise unnecessary escalation and transitions from the preferred place of care, ensure that medications are appropriately prescribed and deprescribed, inform families about care and symptoms, and prevent avoidable hospitalisations or medical interventions.

Learning Outcomes:
1. Identify and recognise stages of resident deterioration.
2. Identify appropriate palliative and end-of-life care tools.
3. Identify actions required to prepare the home, staff, resident, and family for stages of deterioration, advancing disease, increasing decline, the last days of life, and terminal phase.

The End-of-Life and Palliative Care Education and Training Framework is a tool outlining the essential knowledge and skills for healthcare professionals caring for those with end-of-life needs. It consists of three domains: Principles of Practice, Clinical Management, and Leadership/Management. Each domain's learning items are categorised into three expertise levels: Foundation, Consolidation, and Extension, forming a continuum of learning.
