This online event explores what to expect and how to prepare for healthcare transition.
The aim is to help you understand self-management principles, how to get ready for transition and how your transition to the adult system will work.
The session is for young people, and parents/carers of young people, who:
- receive care, or have received care, with the Sydney Children's Hospitals Network, and
- are in the early stages of transitioning to adult services or aged between 14-17 years old.
