Tue. 18 October 2016, 12:15 pm – 3:30 pm AWST
While under pressure, a tank (a pressure vessel) had a catastrophic failure causing the death of two workers.
This workshop examines in detail the background to the event, the cause of the explosion and practical guidance on the inspection of pressure vessels.
OSH advisors, managers and supervisors who deal with plant in the workplace have a unique opportunity to work through an investigation and examine some of the evidence.
Presenter: Geoffrey Kennedy: Plant Inspector, Plant and Engineering, WorkSafe Division, Department of Commerce
Geoffrey is a certified in-service plant inspector with experience in petroleum and mining. He is on the executive committee of the Australian Institute of Engineering Surveyors.
