Hear from Internationally renowned guest speaker Professor Kurt Stange
Professor Stange and the Annals of Family Medicine have had a major interest in the development of the patient-centred medical home in the US. This event will offer insights into what helps to make this model successful.
This session is aimed at GPs, Medical Specialists and Allied Health however all health professionals with an interest in the Medical Home model are welcome to attend.
Light dinner will be provided.
About the Speaker
Kurt C. Stange, MD, PhD is a family and public health physician, practicing at Neighbourhood Family Practice, a federally-qualified community health centre in Cleveland, Ohio, USA. He is a Distinguished University Professor, and is the Gertrude Donnelly Hess, MD Professor of Oncology Research, and Professor of Family Medicine and Community Health, Epidemiology & Biostatistics, Oncology and Sociology. He is working on Promoting Health Across Boundaries (www.PHAB.us), and is active in practice-based, multimethod, participatory research and development that aims to understand and improve primary health care and community health.
