Pathways to Palliative Care
From the number of calls Palliative Care WA receive it is clear that the health professionals and community alike have difficulty understanding the palliative care system; what services are available, who to call and where to go for help when someone is needing access to palliative care. This forum will bring together a panel of agencies that provide information and support services for palliative care inquiries.
Carol Douglas, Aged Care Nurse Practitioner, Residential Care Line Outreach Service, Sir Charles Gairdner Hospital, will lead the forum with a presentation that highlights the outcomes for an older person, with declining health living in a residential aged care setting, particularly in the absence of a ‘clinical treatment plan’.
Dr Elissa Campbell, Geriatrician and Palliative Physician, will put a number of case scenarios to the panel for their advice to map and direct the best pathway to ensure timely access to quality palliative care treatment aligned with patient goals and needs. Also, attendees will have an opportunity to put forward their own scenarios.
