For GPs

This education session will provide GPs with an understanding and increased awareness into updates in medication management, palliative care processes and advanced care directives in relation to older patients.

LEARNING OUTCOMES

  • Identify why complex medication regimens represent a burden to residents;

  • Describe factors that contribute to medication regimen complexity;

  • Apply an implicit tool to identify opportunities for medication regimen simplification;

  • Identify how early recognition of deterioration to trigger palliative care planning can improve clinical outcomes at the end of life;

  • Discuss how the use of practical tools and resources help GPs provide palliative and end of life care to older persons in residential and community settings;

  • Discuss GPs legal and clinical responsibilities regarding advance care directives, including available dispute resolution processes.
