Description

Support coordination is an important role that aims to strengthen capabilities of NDIS participants to implement and achieve all supports in their plans. This includes using informal, mainstream, community and funded supports.
The purpose of the NDS Support Coordination Skill Set Training is the continued capacity building of the disability workforce, with further aims to build knowledge and promote consistency of Support Coordination offered to people living with a disability in Tasmania.
This skill set is aimed at people who are: currently Support Coordinators who want to enhance knowledge and skills; people who are receiving or are able to receive mentorship as Support Coordinators within an organisation, or independently.


Units of Competency

  • CHCCSM005 - Develop, facilitate and review all aspects of case management

  • CHCDIS005 - Develop and provide person centred service responses


Dates for 2018

Tuesday 13th March 2018 – first workshop 10am - 2:30pm

Enrolment, overview of learning & assessment, and introduction to the online learning

Wednesday 14th March – Monday 9th April - Readings

Complete readings and viewings online in preparation for workshop

Tuesday 10th April – Practical Workshop 10am - 4:30pm

Practical Workshop day for ALL learners

May 14th – 18th – Workplace visits

(date and time to be negotiated with assessor)

Workplace visits to discuss projects and learning and onsite assessment

Thursday 31st May - Final Workshop: 10am – 2.30pm

Assessment submission and reflection presentation


Skillset Learning Outcomes:

Upon Skill Set completion candidate should be able to:

  • Provide Support Coordination within the legislated NDIS framework and in accordance with organisation requirements
  • Work with participants with differing goals, needs and aspirations to develop and implement person-centred plans and / or plan reviews
  • Conduct Support Coordination meetings with participants and where applicable, relevant others
  • Monitor, evaluate and review participant progress towards plan achievement
  • Report participant progress towards plan achievement to NDIA
  • Provide a quality service response within specified financial parameters
  • Develop deeper understanding of the definition of 'support coordination'
  • Explore and identify non funded options and strategies in linking to community and mainstream supports
