Description

NSW North Stakeholder team and NSW Dept Health are co-presenting workshops targeted at registered providers of support coordination and allied health.

The workshops will discuss the responsibilities of the NDIS and the health system, and provide tips for service providers on how to work with the health interface.

This workshop will cover:

  • Role of the health discharge worker/planner
  • Role of the support coordinator working with the health system
  • Overview of AT Home Mods and how the support coordinator can work with the health interface
  • Responding to the needs of people with a disability during hospitalisation
  • Case examples

Support coordinators will have an opportunity to speak with NDIA staff as well as representatives from their local Health districts.

It is important to remember that the purpose of the workshop is to enhance but not replace your internal staff training and development and it is expected that your nominated staff will transfer their learnings to their work colleagues.

Light refreshments will be available. Please notify of any dietary requirements and inform us if you have any accessibility, interpreting or special requirements.
