Registration from 9:30am, workshop to begin at 10am-1pm. Please join us for lunch after the workshop.

The PHN is redesigning two programs, the Primary Mental Health Care and Mental Health Nurse Incentive Programs. The redesign is intended to result in the development of a clinical mental health service model that will improve service integration and achieve alignment with a person-centred, Stepped Care approach.

The outcome of this service re-design process will be the development of a model that addresses local need, increases access for hard to reach populations and supports best practice multidisciplinary care.

Through service design, planning and commissioning activities we are seeking to:

  • promote more joined up services
  • encourage the adoption of person-centered and holistic care
  • develop linkages with broader health services that support integrated services and referral pathways.

The re-design of these services will be informed by service providers and community members, including people with lived experience, from across the Hunter New England and Central Coast region.

To this end, we are inviting you and/or nominees to a first workshop for key clinical service providers and members of the PHN Clinical Councils, Community Advisory Groups and Board. The workshop will be facilitated by Nexus Management Consulting, who will be providing more information closer to the event. We will also be holding follow-up workshops in the Hunter, Central Coast and New England areas over subsequent weeks.
