Description

Aim:

This activity explores the importance of needs based care approaching the end of life. It describes how GPs, Nurses, Allied Health and Pharmacists can manage a "good death" by mobilising local resources to contribute to the palliative care team and explores ways to manage common terminal symptoms.

Learning Outcomes:

Explain how to implement a framework of palliative care into routine practice to ensure proactive and best practice end-of-life care.

Recognise the value of advance care planning and apply it to a local context.

Discuss the importance of documenting patient centred medical goals in each treatment plan.

Identify key components of an end-of-life (terminal) phase management plan for community-based patients
