Event description

Description

what does it mean when providing and managing clinical care?

Case Management is a highly skilled specialised role. Providers of human services assist with consumers via case management to have meaningful lives whilst getting the most out of their allocated funding. Case Management is provided in many sectors such as health, aged care, disabilities, home care, mental health, drug & alcohol services. Whilst many case managers are not clinicians they are expected to be able to know when to refer to others and respond to changing conditions in clients.

Providers are facing ever increasing questions about their case management/core advisory services and associated fees. In addition, there is a demand that clinical care is managed effectively.

This seminar will allow participants to reflect on their case management model and assess their risks associated with clinical care.

Key topics covered include:
• describing case management
• principles of case management
• marketing your case management to stakeholders
• how to document services that meet legislative requirements
• costing your services
• how to conduct a case conference
• care planning and assessments required for informed decision making
• making consumers the centre of all practice
• assessing clinical risks
• what is clinical governance and what is required when implementing a framework
Aged care is an ever changing landscape, and as the industry expands there is increased emphasis put on robust clinical care, documentation, case conferencing and operating procedures. A report recently released by the WA Coroner has highlighted the need for clinical governance for Home Care Providers.
To maintain quality standards and to protect your organisation, it is imperative to implement a sound clinical governance framework.

This MUST ATTEND event has been designed to provide organisations with all information about how case management and clinical care can be intertwined.

Lucille Scomazzon from Maddocks law firm will present case studies and the necessary framework to meet legislative requirements. Lorraine Poulos, a Registered Nurse and the facilitator for the day will provide you with guidance and advice about how to meet legislative requirements and discuss changes in the industry and how this affects your organisation. Lorraine will assist attendees to assess their current model and bring your organisation in line with best practice and standards.

Lorraine has worked closely with government and the sector for over 20years and brings valuable insights to the day.

Workshop includes the provision of useful resources to take home on USB.

Tools will be provided to assist participants to implement the changes covering:
• What is clinical governance?
• How does clinical governance protect your organisation?
• Implementing best practice processes
• Quality documentation
• Risk management
• case conferencing
• Common clinical care (wound care, medications, continence issues)
