If you’re having trouble setting individual goals with your clients and/or you’re writing GDCPs but you’re not convinced about their value, this workshop is for you.
This workshop will build on the information provided in the introductory GDCP workshops and focus on how effective goal setting and care planning can really add value to your work with clients. We will discuss the key skills required to effectively engage clients and carers in the GDCP process and work collaboratively to:
- introduce the concepts of goal setting and care planning to clients and carers
- identify meaningful goals
- document relevant and useful care plans (including using appropriate language, strategies to reinforce a strengths based approach and how to ensure key elements can be documented efficiently).
You will have an opportunity to review examples of your own work, seek feedback and discuss your experience of implementation to date.
“Today was a great opportunity to clarify a number of details that I missed in the initial training. I am really starting to hone my skills now and feel much more confident in helping my clients develop plans that are right for them”
“Our team has been working hard on care planning since our first training but it’s so valuable to check back in with a true expert, have our questions answered and reinvigorate us”
“Great session! Full of practical tips I can use to keep on improving my practice”
“I found the first training enlightening but when I got back to work, I got stuck sometimes trying to make it all work with my clients. This session was so helpful in allowing me to bring it all together and discuss some of the stumbling blocks I’ve come up against along the way. Thank you!”
