Description

The Making Links project is a partnership between the AOD, mental health and homelessness sectors in Melbourne's north and west. The project aims to improve coordination and linkages across these sectors for the benefit of shared clients.

In December 2015 the first Making Links forum was held, bringing the sectors together to create a shared understanding of how clients are accessing services, how each service system operates and providing an opportunity to explore how to improve service coordination.

Participants in the Forum identified priority areas for this work in 2016. One of the key priorities identified was the need to better understand each other's Sectors. To assist with this the Making Links partnership has developed an Orientation Kit.

The Making Links Orientation will provide practitioners with an overview of navigating each of the three Sectors, an outline of the NDIS and an opportunity to network with practitioners from other Sectors.

The Forum is targeted to new practitioners from the AOD, mental health and homelessness sectors in Melbourne's north and west.

Morning tea will be provided.
