Description

This program is suitable for adults.

Do you have a chronic health condition? A chronic condition is defined as any medical condition lasting six months or longer, for example asthmas, arthritis, cancer, obesity, diabetes, chronic pain and heart disease.

This 6 session course is led by a Health professional and a person with a chronic health condition. The course enables individuals to learn strategies to help manage your health conditions including pain & fatigue management, action planning, problem solving, healthy eating, exercise, and relaxation, over 6 weeks.

This course is a 6 week course. It will be at Belconnen Library from Wednesday 19 July to Wednesday 23 August. All sessions are 10.00am-12.30pm.
