(Symptoms include: Fever >37.5 degrees or history of fever (chills, aches, night sweats), cough, sore throat, shortness of breath, runny or stuffy nose and/or loss of taste and/or smell)
3. Have you, your child, or attending support person been in close contact with a confirmed case of COVID-19 in the last 14 days?
4. Are you or any of your household members waiting for the COVID-19 testing results?
5. Have you, your child, or attending support person currently required to self-quarantine or self-isolate for any reason?
6. Were you on or in close contact with a passenger on Virgin flight VA962 Brisbane to Sydney on 17 August 2020 (Arrival time: 5.40 pm)?
