The 2019 inaugural Murray PHN General Practice Investment Strategy signalled a new way of commissioning work within general practice which we are pleased to continue in 2020. We have funding packages and service-based opportunities that include:

• Care coordination for patients with chronic disease

• Improving access to after hours services

• Mental health services for hard-to-reach populations

• Chronic disease remote patient monitoring

• Supporting mental health and AOD services in general practice
