Taylor Made Outcomes is a registered provider of Specialist Support Coordination, with a strong passion and focus on everyone being supported to have a safe and appropriate home.
We have successfully achieved Specialist Disability Accommodation (SDA) for 5 participants and assisted in the transition of multiple participants from aged care or hospital into secure and permanent housing with appropriately funded supports.
Beck Corley and Mandy Drain have developed a Housing Manual and supporting framework to guide Taylor Made Outcomes Support Coordination team on approaching any housing avenue, including NDIS funded streams.
We are offering the opportunity to see the framework we have built and our session will cover:
1. Housing - determining if NDIS is the most appropriate model of funding
2. Social and Affordable Housing
2. Specialist Disability Accommodation (SDA)
3. Supported Independent Living (SIL)
4. Medium Term Accommodation (MTA)
5. Individualised Living Options (ILO)
6. Applying for multiple housing streams - why you must explore more than one housing pathway
While we wont be offering the manual for distribution, we believe that our session content, which will be presented in a face to face setting with PowerPoint slides, will assist you to understand the interface between Housing and the NDIS.
