Join us for a case study education evening to support General Practitioners,

QAS and community health professionals to manage the complex care of patients

and their families with a life limiting illness in the community

 

with Dr Louise Welch

from the SCHHS Palliative Care Consult Liaison Team

including panel discussion with

Little Haven, SCHHS and local Gympie GP


Case Study Learning Outcomes:

  • Discuss symptom management for a person with a life limiting illness in the community;
  • Overview of advanced care planning and advanced care directives;
  • Identify strategies for initiating difficult conversations about end of life care.

 

Free event and three course meal included (funded by CPCRE grant)

 

This activity has been approved by the RACGP QI&CPD Program for 4 (Category 2) points

(Activity no. 165205) in the 2017-2019 Triennium. ACRRM points pending.

It is one of 31 Commonwealth-funded PHNs nationwide.

  • Coordinate primary health services to meet the needs of our communities, particularly those with high health needs
  • Improve the patient journey between primary and acute care
  • Support general practice
  • Improve access to health services, especially for people in remote areas
  • Ease pressure on hospitals.
