New general practice funding packages

Murray PHN will provide $3 million to general practices in 2019 to provide new or additional services for patients in our region. We have funding packages that include:

• care coordination services for patients with chronic illness
• alcohol screening and brief intervention programs
• after hours networks, and
• low intensity mental health services for children.

We are holding workshops in each region to outline the opportunity for general practices and how to apply. In addition, each workshop will include an introduction to care coordination and business modelling for team-based clinics.

• Shepparton, 19 February 2019
• Bendigo, 20 February 2019
• Mildura, 21 February 2019

Workshops are aimed at decision-makers in the general practice, e.g. practice managers, GPs or practice nurses involved in care coordination. Places are limited to 2 people per practice.
