Session Content:
Health professionals and aged care workers aim to provide care that is patient-centred and delivered in line with a person’s goals, beliefs and preferences, particularly as people age and become more likely to have multiple chronic conditions. Decision Assist is delivering a series of free workshops in South Australia, in partnership with Adelaide PHN and Country SA PHN. Workshops will improve knowledge, skills and confidence in advance care conversations.
- What are the benefits of advance care directives (ACDs)?
- What are your roles and responsibilities?
- How can you communicate effectively about future medical treatments, ACDs and end-of-life care?
- How can you incorporate ACDs into your workplace and routine care?
- What are the legal requirements?
All health care professionals from general practice, aged care, allied health and community agencies are invited to attend these interactive sessions. These workshops are being presented in the metropolitan areas of Adelaide, as well as regional South Australia.

Delivering an extensive range of projects, ACPA increases advance care planning resources across health sectors and NGOs, improves workforce capability, produces information resources for diverse consumers and communities, and builds the evidence base.
