From 1 November 2025, the Support at Home (SaH) Program now includes two new short-term care pathways: Restorative Care and End of Life Care. All SaH providers are expected to facilitate these pathways for clients approved for these funding streams.

This 90-minute webinar will guide you through the practical considerations and steps required to establish service models for these two pathways within your organisation.
