Description

End of Life Care is a well-recognised priority area, both nationally and locally with Australia’s rate of dying at home being comparatively low compared with other similar OECD countries. Whilst acknowledging that people want to be able to die in their place of choice, there are numerous factors that preclude this from happening for a lot of people.

The Eastern Melbourne Primary Health Care Collaborative (EMPHCC) Alliance have identified End of Life Care as a priority project for 2017/2018.

Using Eastern Health’s Rapid Improvement Methodology this workshop will consider how many people die in their place of choice and for those that don’t, the reasons why it doesn’t happen.

The workshop will form part of the investigation the End of Life Care Working Group are undertaking into understanding the barriers people have in dying in their place of choice and will help inform the co-design of potential solutions to ensure more people are supported in their choice of where they wish to die.
