DAY ONE - Embedding a Wellness appraoch in Assessment and Planning

This interactive workshop will support staff to build on their assessment and planning skills and consider ways to conduct more meaningful, person centred assessments, and create service specific care plans that add value for staff, clients and carers.

This will include discussion of how to:

  • Embed a wellness and reablement approach in your practice
  • Empower consumers to make informed decisions about their health and healthcare
  • Understand what’s important to clients, their needs and priorities.
  • Utilise information from the My Aged Care portal to inform and streamline their service specific assessments, reduce duplication and improve consumer experience and efficiency
  • Engage clients and carers to identify relevant and meaningful goals
  • Document relevant and useful service specific care plans (including using appropriate language, strategies to reinforce a strengths based approach and how to ensure key elements can be documented efficiently)

Staff will have an opportunity to review examples of their own work, seek feedback and discuss their experiences with colleagues.

Target group:

This workshop is appropriate for both new and established staff who are actively involved in assessment and planning.

What to bring: Attendees are to bring:

  • Their current assessment tool/s (can include any templates that are used to collect / document client information during the intake and assessment process)
  • An example of a care plan they have created with a client. These can be documented on whatever template the service uses to document client goals and plans. Wherever possible, care plans should be de-identified to protect client confidentiality.

The Resources include:

  • The GDCP Toolkit
  • Full colour handouts and training materials for each participant
  • The Service Specific Assessment & Planning information sheets including:
  • Reducing duplication in service specific assessments
  • Embedding a strengths based approach in client conversations
  • Service specific care planning, and Monitoring
  • Feedback & Review of service specific care plans


DAY TWO - half day - Supporting Wellness and Assessment and Planning in Your Agency

Program Overview: This program focusses on how agencies can develop and embed the organisational systems and tools required to support their staff embed effective, service specific assessment and planning.

This will include information about:

  • The key elements of a best practice approach to assessment and planning and strategies to support ongoing skill development among practice staff
  • The expectations outlined in the Aged Care Quality Standards and CHSP practice guidelines
  • How to develop relevant organisational Policies and Procedures (in line with relevant standards)
  • A framework to support agencies evaluate assessment and planning tools and approaches (including the presentation of practical audit tools, checklists and guidelines regarding collecting feedback from staff, clients and carers).

Target Group:

Staff responsible for the development and implementation of systems to support service delivery. This may include team leaders, program managers and quality improvement staff.

Staff that attend this program should also attend the full day session – ‘Embedding a Wellness Approach in Assessment and Planning’. Quality improvement staff who are involved in the system development, but do not have any direct link to practice staff are not required to attend the full day session.
