This workshop is designed specifically for staff working in group settings. We will discuss the unique elements of care planning in a group program and how you can work together to document meaningful, individualised care plans for each member of your group. The session will include information about:
Making individualised goals meaningful within group activities
- The key elements that need to be documented in GDCP and how these can be documented effectively and efficiently.
- Using appropriate language and strategies to reinforce a strengths based approach within GDCP documentation.
- Incorporating different perspectives within a GDCP (e.g. client, carers and staff).
The workshop will utilise a range of case studies and participants will also have an opportunity to discuss their experience and seek feedback on examples of their own work.
“Today was so useful, thank you. How to write the plans is so much clearer for me now”
“Thanks for a wonderful workshop. The practical activities have cemented exactly what we need to do and going through it step by step made it seem much simpler than I thought it was this morning”
“It was very helpful to understand the difference between care planning and activity planning. I can see now why setting goals with my members will help us all work better”
